The innocence Project is a national [USA] litigation and public policy organization dedicated to exonerating wrongfully convicted people through DNA testing and reforming the criminal justice system to prevent future injustice.

The main causes of wrongful conviction are listed as:

Bad lawyering

New Commission on Wrongful Convictions in NY
On May 1, the Chief Judge of New York’s highest court announced the creation of a permanent Justice Task Force to study the causes of wrongful convictions and develop systemic remedies to improve the state’s criminal justice system. The Innocence Project called the move a “huge step forward” and a national model.
